If you would like to temporarily block your account from further use due to either losing, misplacing, or having your library card(s) stolen, please accept the terms of our Lost/Replacement card policy and click on “Submit Lost Card” to process your request.
Please read the following information as it pertains to the restrictions you will have when reporting your card lost/misplaced/stolen:
- By clicking on the “Submit Lost Card” button, you are finalizing your decision to temporarily block your account from further usage as it pertains to borrowing library materials, eBooks/eAudio, and utilizing library technology.
- If you have any materials checked out, you will no longer be able to renew your borrowed materials. You must visit your nearest Brooklyn Public Library and speak with a staff member to obtain a replacement library card.
- Reporting your card lost/misplaced/stolen does not affect your ability to pay accrued fines and fees in your online account.
- Any items that were borrowed and/or fees accrued prior to reporting your card lost or stolen remains your responsibility.
- There is a replacement card fee of $2.00 for Adults and Young Adults; $1.00 for Children and Older Adults.
- If your account has already been blocked by the library due to referral to the collection agency, bounced checks or failure to update your personal information, you will not be able to utilize this feature.
- Once you have submitted this form, you will not be able to unblock your account online. You will need to visit your nearest Brooklyn Public Library and speak with a staff member to obtain a replacement library card.
- To obtain a replacement library card, you must bring with you at the time of your visit a valid and current photo ID with address. For alternative IDs that may be used to fulfill the identity and residency requirement, please visit https://www.bklynlibrary.org/use-the-library/borrow.
